چکیده

Background: Pregabalin is efective both at controlling postoperative pain and preventing chronic neuropathic pain. Local anaesthetic iniltration is a simple and inexpensive method to provide postoperative analgesia. In this study we tested the hypothesis that pregabalin administered perioperatively combined with continuous wound infusion will aid better the post-thoracotomy pain management compared to pregabalin monotherapy. Methods: Forty ive patients received either placebo (PLCB), pregabalin (PRG) or pregabalin and continuous wound infusion (PRG + CWI) of local anaesthetics. Postoperative data collection included opioid consumption, VAS scores at rest and during cough. At 1 and 3 months from surgery patients were assessed with the DN4 questionnaire for neuropathic pain. Results: VAS scores were signiicantly lower in the PRG+CWI group (p < 0.05) at rest while during cough the placebo group had higher scores than both treatment groups (p < 0.001 or p < 0.05). Morphine consumption measured at 48 hr PO revealed a signiicant diference in total morphine; PLCB: 49 ±11mg, PRG: 33 ±10mg and PRG+CWI: 28 ±11mg (p < 0.001 between placebo and the other two groups). The incidence of neuropathic pain was more frequent in the placebo group (1-month: PLCB: 10 pt, PRG: 0 pt and PRG+ CWI: 0 pt (p < 0.001), 3 months: PLCB: 10 pts, PRG: 0 pt and PRG+ CWI: 0 pt (p < 0.001)). Conclusions: Perioperative administration of pregabalin signiicantly reduces pain scores, opioid consumption and incidence of neuropathic pain in post-thoracotomy patients. The addition of continuous wound iniltration of local anaesthetics although improved immediate postoperative analgesia at rest did not afect diferently the development of neuropathic pain. Do surgical patients benefit from perioperative gabapentin/pregabalin? A systematic review of efficacy and safety.

BACKGROUND Gabapentin and pregabalin have antiallodynic and antihyperalgesic properties useful for treating neuropathic pain. These properties may also be beneficial in acute postoperative pain. In this study we evaluated randomized, controlled trials examining the analgesic efficacy, adverse effects, and clinical value of gabapentinoids in postoperative pain. METHODS A systematic search of M...

The effects of preoperative oral pregabalin and perioperative intravenous lidocaine infusion on postoperative morphine requirement in patients undergoing laparatomy

OBJECTIVES To evaluate and compare the effects of preoperative oral pregabalin and perioperative intravenous lidocaine infusion on postoperative morphine requirement, adverse effects, patients' satisfaction, mobilization, time to first defecation and time to discharge in patients undergoing laparotomy. METHODS Eighty patients (18 to 65 years of age) undergoing elective laparotomy were randoml...

The analgesic efficacy of celecoxib, pregabalin, and their combination for spinal fusion surgery.

BACKGROUND As optimal pain relief after surgery is difficult to achieve with the use of just one drug, many pain experts advocate the use of two or more classes of medications so as to reduce the side effects from any one drug. In this trial, we assessed the analgesic efficacy of administering perioperative celecoxib, pregabalin, or both after spinal fusion surgery. METHODS Eighty patients sc...

Perioperative gabapentin for the prevention of persistent pain after thoracotomy: a randomized controlled trial.

OBJECTIVES To evaluate the effect of perioperative gabapentin treatment for the prevention of persistent post-thoracotomy pain and to establish whether gabapentin has a significant therapeutic impact on acute postoperative pain. METHODS Consecutive patients with pulmonary malignancies scheduled for anterior thoracotomy were enrolled in this randomized, double-blinded, placebo-controlled trial...

Perioperative oral pregabalin reduces chronic pain after total knee arthroplasty: a prospective, randomized, controlled trial.

BACKGROUND Despite the enormous success of total knee arthroplasty (TKA), chronic neuropathic pain can develop postoperatively and is both distressing and difficult to treat once established. We hypothesized that perioperative treatment with pregabalin, a chronic pain medication, would reduce the incidence of postsurgical neuropathic pain. METHODS We performed a randomized, placebo-controlled...
